Temporary measure to renew the Health Insurance Card

To renew your Health Insurance Card, you must mail your completed and signed renewal form to us, without going through the Société de l’assurance automobile du Québec service outlets. You do not need to provide a photo: we will use the previous photo available on file. This is a temporary measure.

How to renew your Health Insurance Card:

  1. Complete and sign the renewal form that you received in the mail.
  2. Do not attach any photo. We will use the previous photo available on file.
  3. Mail your application to the address indicated on the renewal form.

No photo on you current card

If you are between 14 and 74 years old and do not have a photo on your Health Insurance Card, you must go to a service point (CLSC) or a SAAQ service outlet to renew your card. You will receive a new renewal notice in 2 years. This does not apply to persons who are exempted from having to provide their photo or signature, for example, due to medical reasons.

Expired card

If your card has been expired for more than 6 months, call us to obtain a new one. If it has been less than 6 months, use the renewal form. In any case, you do not have to go through the Société de l’assurance automobile du Québec service outlets.
