Procedure for replacing a damaged card

Protection of your personal information
In order to offer you certain services, we need to collect, use, and sometimes disclose your personal information. To find out more about our privacy practices on our website, please refer to our Privacy policy webpage.

To file your application online, you must:

  • be submitting the application for yourself or your children under age 18 who are living at your address (after your authentication, you will be able to choose the child for whom you want to replace or correct the card)
  • be aged 18 or over
  • not be under tutorship or represented by another person
  • have a Government Authentication Service account or a clicSÉQUR account, or know your Health Insurance Number to create an account (otherwise, you must send your application by mail)

By mail

Order a card replacement form online or contact us to obtain it. You will receive the form by mail. Upon returning it, you will have to pay a $25 fee, either by cheque, bank draft or money order.

Persons age 65 or over, and recipients a financial assistance program with claim slip (carnet de réclamation)

The replacement of a damaged card is always free for persons age 65 or over, and for recipients a financial assistance program with claim slip (carnet de réclamation).

The new card will include the same photo, signature and date of expiry as those on the card being replaced.

Replace your Health Insurance Card online

Has your Health Insurance Card been lost, stolen or damaged?

No problem. You can replace it online.

But first, make sure you have a Government Authentication Service account.

And if you’ve moved recently, double check that you changed your address, so your card is delivered to the right place.

Then, click on “Apply online”.

Identify yourself using the Government Authentication Service.

Once you’re logged in, enter the required information.

Select the person who needs to replace their card and the reason for replacement.

Finalize your transaction and voilà, your request is sent!

Change of address

It is important to make any changes of address before completing your application. You may do so online using the Service québécois de changement d’adresse.

Card delivery time

We will mail your new card within 2 weeks after we’ve received your complete application. This can take up to 3 weeks when we receive a high volume of applications. Until you receive your card, contact us if you need services that are covered under healthcare.

Identity theft

If you believe that you were a victim of identity theft after your Health Insurance Card was lost or stolen, contact us. To find out what steps you need to take, refer to the Vol d’identité page on the website of the Commission d’accès à l’information du Québec.
