We will be carrying a maintenance of our online services from 22:00 p.m. on April 4 to 8:00 a.m. on April 5. Our online services will be unavailable, including the following: registration for and deregistration from the prescription drug insurance, replacement of a Health Insurance Card, organ donation, and advance medical directives. You can use this form to send us documents at our request. This is the case if we have asked you to do so by mail or telephone.

This service is only available to you if we have asked you to send us documents online.

You received a RAMQ document by mail

We sometimes request supporting documents to complete the analysis of an application. This may be a missing document to justify a status, or proof of studies or work, for example. We can also send you a form to complete, scan and return online. The forms concerned are as follows:

  • Notice of Return to Québec
  • Notice of Taking Up Residence Outside Québec
  • Temporary Departure from Québec
  • Birth Outside Québec

In all cases, check the mailing instructions on the document you receive.

You spoke on the phone with a RAMQ agent

Has a RAMQ agent asked you to send a document using this online form? You will need to enter the number the agent gave you on the phone and attach the requested documents.

Before filing your application

Based on your situation, make sure you have:

  • A scanned copy of the document received by mail, if applicable. A barcode is located at the top of this document.
  • A scanned copy of the supporting documents requested, if applicable.
  • The number(s) provided by the person you spoke to on the phone, if applicable.

Tips for scanning attachments

To scan a document, you may use a scanner or a scanning application on your smartphone. You must save the document in PDF or image (JPG) format.

To take a photo of your document and send us legible information:

  • Place the document on a fixed horizontal surface, such as a table. Avoid holding the document in your hands.
  • Make sure you have enough light to avoid dark areas and shadows in the photo. Do not use your camera’s flash.
  • Position the camera parallel to the surface and at a sufficient distance to frame the entire document.
  • Avoid moving while taking the photo.
  • Make sure that the photo is clear and that all the information in the document is visible.
