Renew a Health Insurance Card Procedure for renewing your Health Insurance Card Step 1 Complete your renewal notice, making sure to check the accuracy of the information already entered. You will automatically receive the notice approximately 3 months before your card expires. If your address has changed, use the Service québécois de changement d’adresse to report it. If you need to have information corrected on your card (first name, last name, date of birth or sex), please refer to the webpage Replace or correct a Health Insurance Card. When you fill out your notice, pay special attention to the following: Temporary absence from Québec Indicate whether you were absent from Québec 183 days or more in the 12 months following your eligibility start date. This date appears in your renewal notice under the section “Absences from Québec.” After these initial 12 months, your absences will be calculated on a calendar year basis (January 1 to December 31). To arrive at the correct number, add all days of absence in a given year, excluding: Absences of 21 consecutive days or lessThe days on which you leave and return Signature Read the declaration and undertakings section carefully and then sign the renewal notice. Indicate the current date and your phone number. The renewal notice of a person under age 18 must be signed by the mother, father or tutor. If you are completing the notice on behalf of someone else, for instance, your parent, make sure that you are authorized to do so. If you have a photo on your current card, you do not need to provide a photo. We will use the previous photo available on file. All you have to do is send us your completed and signed renewal notice. Are you between 14 and 74 years old and do not have a photo on your current card? You must go to a health network service point (CLSC). This does not apply to persons who are exempted from having to provide their photo or signature, for example, due to medical reasons. You must have the following documents with you: Your completed and signed renewal notice2 pieces of ID (including one with a photo)A photo If you are 75 years old and older, you are not required to provide your photo and signature to obtain your card. If you do not have a photo on your current card but wish to have one, you must go to a health network service points (CLSC) to renew your card. If you have misplaced your renewal notice, you may order the form online. You will receive it in the mail. Card delivery time We will mail your new card within 2 weeks after we’ve received your complete application. This can take up to 3 weeks when we receive a high volume of applications.
